¿Puede y debe ser sustituida la clasificación de la ILAE por una clasificación basada en la semiología?

RESUMEN
Introducción: La clasificación actualmente aceptada por
la Liga Internacional Contra la Epilepsia no es una clasificación
semiológica de las crisis epilépticas.
Objetivo:
Determinar si es posible obtener una clasificación
semiológica de las crisis epilépticas a partir del interrogatorio
y con ello estimar la localización y lateralidad
de la zona epileptógena.
Métodos: Se realizó un estudio
multicéntrico descriptivo transversal a doble ciego a
189 pacientes remitidos a evaluación prequirúrgica en
el periodo de los años de 2007 al 2011. Se les realizó una
entrevista estructurada y video-telemetría.
Resultados: Las
auras, convulsiones focales, disfasia, psicosis ictal, confusión
postictal, versión de la cabeza, cefalea
hemicraneana y amnesia preictal permitieron diferenciar
el origen de las crisis. El orden temporal en que aparecen
las auras temporales típicas, las crisis focales frontales,
seguidas de hemiconvulsión focal o generalizada,
o con automatismos en miembros inferiores sin
automatismos oroalimentarios, asociados a confusión,
depresión y la existencia de un período de latencia
permiten diferenciar claramente las zonas epileptógenas.
Conclusión: Los síntomas perictales evaluados por video-
EEG pueden ser obtenidos por la entrevista clínica, los
que asociados a los síntomas interictales y analizados en
estricto orden temporal permiten estimar la localización
y lateralidad de la zona epileptógena.
