Sodio hipertónico nebulizado en el tratamiento de bronquiolitis comparado con manejo convencional

ABSTRACT
Introduction. Bronchiolitis is the most frequent respiratory tract infection on children ‹ 2 years old, in the 80% of the cases the
etiological agent is the human respiratory syncytial virus (RSV). Either in the winter or in the early springs its cosmopolitan
distribution and epidemic course takes place. There are some controversies about which of the treatments are the best. Today, the
best therapies are hydration and oxygen therapy. The hypertonic salt solution has been a therapy option.
Objective. To evaluate
if the use of nebulized hypertonic sodium is effective, since there is evidence of its effectiveness in previous studies.
Material and
methods. Patients from 2 to 24 months who were admitted in paediatrics as a matter of urgency, with diagnose of bronchiolitis.
They were evaluated through the modified Respiratory Distress Assessment Index (RDAI), with 4 as maximum score and without
treatment for cardiopulmonary disease. Two groups were made: Group 1, patients with salt solution of 0.9%; Group 2, hypertonic
sodium of 3% in micronebulizations every 4 hrs.
Results. There were 32 patients, being more males than females; 71%
vs. 29%,
the 93.7% was between 2 and 12 months old, 17 patients in the group of salt solution of 0.9% with an average RDAI score of 5.7.
They needed 12.6 nebulizations to have a RDAI score of 0. In the group of hypertonic sodium were 15 patients, with an average
score of 6.7, they needed about 5.1 micronebulizations in order to get a RDAI score of 0.
Conclusions. Despite the small number
of patients, the study was statistically significant. The use of hypertonic solution reduced as much the number of micronebulizations
as the days of intrahospitality stay. As a result, the hypertonic solutions are effective for treating bronchiolitis with success.
