ABSTRACT
Bacterial meningitis (BM) is a medical, neurological, and sometimes neurosurgical emergency that requires a
multidisciplinary approach. Has an annual incidence of 4 to 6 cases per 100,000 adults, and his mortality is very high
perhaps to count with antimicrobials effective. The classical clinical manifestations of bacterial meningitis in adults are
fever, headache, neck stiffnes, and signs of cerebral dysfunction, and usually are present in more than 80% percent of the
cases. The cerebral dysfunction is to show by confusion, delirium or reduce of level consciousness. The diagnosis is based
on the cerebrospinal study. Gram’s staining of cerebrospinal fluid allow the rapid identification of the causative organism
in 60 to 90 percent of the cases. The initial treatment approach to the patient with suspected acute bacterial meningitis
depends on early recognition of the meningitis syndrome, rapid diagnostic evaluation, and emergent antimicrobial and
adjunctive therapy. The empirical therapy is based on the cephalosporin plus vancomycin administration while the culture
of cerebrospinal fluid is obtained. The adjuvant therapy with dexamethasone reduce the frequency of neurologic sequelae
among survivors.
