Chronic Achilles tendon rupture. Surgical treatment

ABSTRACT

The objective of the present study is to report the experience from the Ankle and Foot Service of the Hospital of Orthopedics "Victorio de la Fuente Narváez" in treating chronic Achilles’ tendon ruptures through the described technique and analyzing its functional results in a retrospective way. The study was performed on patients diagnosed as presenting chronic Achilles’ tendon rupture of 4-week evolution, who were practiced a restorative plasty using the fibrous tissue interposed in the lesion. From August 1997 to October 1999, 14 patients were evaluated, 12 males and 2 females, with an average age of 39.2 years, an average period of lesion evolution of 14.8 weeks and an average following time of 10.9 months. The results were evaluated by Leppilahti’s functional scale, obtaining excellent results in 57.1% of the cases, good in 42.9% of the remaining cases, without finding poor or bad results. The complication index was of 28.5%, which did not interfere with the functional final results. We consider that, in spite of the few cases and the short following period, the technique is a good alternative for treating this type of lesion, since employing a vascular and thick-enough structure such as the interposed fibrous tissue, assure us a stable and reliable restoration which let us begin the patient rehabilitation and an early recovery.
