Ruptura crónica del tendón de Aquiles. Tratamiento quirúrgico

RESUMEN
El objetivo del presente trabajo es dar a conocer la experiencia del Servicio de Tobillo y Pie del Hospital de Ortopedia "Victorio de la Fuente Narváez" en el tratamiento de las rupturas crónicas del tendón de Aquiles mediante la técnica descrita, analizando en forma retrospectiva los resultados funcionales de la misma. El estudio se llevó a cabo en pacientes con diagnóstico de ruptura crónica del tendón de Aquiles de 4 semanas de evolución, a quienes se les practicó una plastía de reparación utilizando el tejido fibroso interpuesto en la lesión. De agosto de 1997 a octubre de 1999, se evaluaron 14 pacientes, 12 hombres y 2 mujeres, con edad promedio de 39.2 años, con un tiempo de evolución de la lesión promedio de 14.8 semanas y un seguimiento promedio de 10.9 meses. La evaluación de los resultados se realizó bajo la escala funcional de Leppilahti, obteniendo excelentes resultados en el 57.1% de los casos y buenos en el 42.9% de los casos restantes, sin encontrar resultados regulares o malos. El índice de complicaciones fue de 28.5%, las cuales no interfirieron con los resultados funcionales finales. Consideramos que a pesar de nuestros pocos casos y el corto período de seguimiento, la técnica es una buena alternativa en el tratamiento de esta lesión, ya que el utilizar una estructura vascularizada y de buen grosor como es el tejido fibroso interpuesto, nos asegura una reparación estable y confiable que nos permite iniciar una rehabilitación y una recuperación temprana del paciente.
