ABSTRACT
We present the case of a 21 years old woman with excessive sweating on hand palms with exacerbation during stress periods and
an evolution of a year. She was treated with aluminum-based antiperspirants, botulinium toxin intradermic injections and anti
anticholinergics having no improvement, consequently was decided to perform a video assisted sympathectomy, presenting
complete resolution of the palmar hyperhidrosis. The essential hyperdrolisis consists of an increased eccrine perspiration above
8-15 mcg/cm2 min, mostly on palmar level. Affects roughly 1-2.8% of the population. Its etiology is unknown even though
many theories are made, the most accepted one establishes an increased activity in an cerebral cortex area. The management of
mild or moderate consists in the application of topical agents such as aluminum salts, iontophoresis, oral anticholinergic drugs
and botulinium toxin type A. The video assisted thoracoscopic sympathectomy (VATS) is indicated in the acute hyperhidrosis
and on patients with repercussion in their psychological, professional and social spheres.
