CPAP adherence in adult patients with severe obstructive sleep apnea syndrome in Mexican population

ABSTRACT
Background: The obstructive sleep apnea syndrome is a risk factor for cardiovascular conditions and diabetes mellitus, and it is related to a decrease in quality of life. One of the therapeutic choices is the continuous positive airway pressure (CPAP) device. The improvement depends on the CPAP adherence, and there are factors that may modify the adherence to the treatment.
Objective: To know the adherence to treatment with CPAP of Mexican adult patients with severe obstructive sleep apnea syndrome, as well as the degree of diminishing of sleepiness during awake.
Patients and methods: A prospective, longitudinal, observational, not randomized trial was designed, which included adult patients (male and female, from Mexico City) with diagnosis of severe obstructive sleep apnea syndrome, with split night polysomnography. A telephone interview was performed asking information regarding the adherence to CPAP and clinical improvement.
Results: Sixty-three patients were included. Side effects found were pharyngeal sore (23.8%), mask related complaints (22.2%), otalgia (9.5%), and headache (6.3%). The 38.1% of the patients presented no complaints. For the Epworth somnolence scale scores, we used a Student t test for related samples, which showed an Epworth media previous to treatment of 13.6 and after treatment of 5.4.
Conclusions: The results showed that the adherence to CPAP in severe obstructive sleep apnea syndrome patients is high, and that proper indications to the patient during the office interview and during the polysomnography are very important. New studies are needed, in order to analyze other populations, such as mild and moderate obstructive sleep apnea syndrome patients.
