ABSTRACT
Fine needle aspiration biopsy has been used with success to diagnose liver tumors of different type. Its use is now common and widespread. Objective: To learn the utility of liver fine needle aspiration biopsy at the Mexico City General Hospital. Method: 39 cases with clinical follow up or histological confirmation were selected from the files of the Pathology Unit of the Mexico City General Hospital over a 21 years period.Six cases were excluded because the smears were inadequate. Two cytopathologists reviewed the smears and a liver pathology expert reviewed the tissue sections (gold standard]. Results: 21 (63.6%) cases were malignant, 12 (36.3%) cases were benign. One case was a false positive and 1 case was a false negative. Sensitivity and specificity obtained were 95.2% and 91.6% respectively, with PPV and PNV of 95.2% and 91.6%. Diagnostic accuracy was 93.9%. Conclusions: Diagnostic accuracy is based on adequate smears, strict morphologic criteria and reviewer’s experience. Caution must be taken when evaluating nodular regenerative lesions and metastatic tumors which can be confused with liver carcinoma. Tumor markers are needed in some cases.
