Ano-rectal cytology: Diagnostic utility

Introduction: Ano-rectal cancer has increased: it is estimated that in 2004 there will be 4010 new cases (American Cancer Society). A new population has arisen due to the risk factor of infection with HIV, particulary in relation to homosexuality. Objective: To assess the use of cytology in the diagnosis of ano-rectal disease. Material and methods: This is a prospective and descriptive study. The patients were those attending the coloproctology service of the General Hospital of Mexico from March to November 2004, including those at risk of both sexes, especially homosexuals and those practicing anal intercouse, and any with lesions requiring ano-colonoscopy and biopsy. Results: Of 63 patients, there were 21 women and 42 men, nine of the later being HIV positive, four of them were homosexual and one bisexual, their ages ranging between 18 and 87. The cytology diagnosis were compared with 31 cases with histopathology diagnosis, the biopsy is considered the gold standard study. The cytology study showed a specificity of 90%, a sensitivity of 83%, positive predictive value ( PPV ) of 89%, negative predictive value ( NPV ) of 84%, false negative ( FN ) of 16%, false positive ( FP ) of 9.6%, prevalence 0.5% and diagnostic accuracy of 87%. Conclusions: A cytological evaluation can be used for a rapid diagnosis of ano-rectal cancer, being accurate in the majority of cases.
