Experience in the embodiment of coasting for intrauterine insemination

ABSTRACT
Background: Ovarian stimulation during IVF is a common practice, it conveys risks such as ovarian hyperstimulation syndrome, multiple pregnancies, among others. Coasting is used to lower risk of hyperstimulation syndrome in patients at risk.
Objective: We reviewed 15 files of patients who were at risk of hyperestimulation syndrome during intrauterine insemination in which coasting was performed, and determine pregnancy rates, duration of ovarian stimulation, days of coasting, outcome of follicles.
Material and Methods: retrospective study from January 2008 to may 2011 where 15 files were identified at the Instituto Nacional de Perinatologia Isidro Espinosa de los Reyes.
Results: 15 patients were identified, Coasting was performed on any day of stimulating if 6 or more follicles measured 14 mm. None got pregnant, 9 of the cycles were cancelled for low response, 6 applied hCG, coasting was used for 1 day in 7 cycles, 2 days in 3 cycles, 3 days in 2 cycles, and 7 days in 1 cycle. Follicle size diminished in all but one cycle.
Conclusion: Coasting is a good tool for IVF for preventing ovarian hyperestimulation syndrome, in intrauterine insemination cycles we could observe follicles diminished size, but we could not prove its efficacy, observing no pregnancies. We need more patients and a prospective randomized protocol for results to be valid.
