Application of the minimally invasive thoracic surgery. Experience and preliminary report

ABSTRACT
The present report is the preliminary result of the minimally invasive thoracic surgery performance at the Military Central Hospital in Mexico City. In a year period, 38 cases were recorded and included all patients that underwent to thoracoscopy or video assisted thoracic surgery. We recorded different variables to establish the next results: female sex was predominant in 22 cases, the average age was 46.7 years with the age range in 16-78 years. 73.6% was reported to benign diseases and 26.4% malignant. The most frequent procedure was diagnostic in 55%, therapeutic 32% and combined in 13%. Pulmonary biopsy in 34%, pleural 10% and mediastinal 10%. The morbidity rate was 5%, mortality 0%, conversion 2%, Surgery time rate was 1.6 hours SD ± 0.74. The kind or disease or organ involved were pleura 39%, lung 36%, mediastinum 13% and combined 12%. Only one case was video assisted thoracic surgery. We consider that the procedure showed advantages, being reproducible and feasible in our hospital. The obtained results were similar to those reported in other series. Thoracoscopy showed a very good tolerance in terms of postoperative pain, is an excellent procedure to access solitary pulmonary nodules, and to drain chronic pleural collections, were the procedure showed the most important clinical impact and finally the feasibility to perform advanced therapeutic procedures like mediastinal masses resections.
