Gallbladder disease as a manifestation of Weil's disease. Report of a case where a resident of Mexico City

ABSTRACT
Leptospirosis is a worldwide zoonosis disease. The 90% of the cases corresponds to the anicteric form and 10% of the cases to the icteric form or Weil's disease.
One of the manifestations of Weil's disease is in the liver which is characterized by increased serum bilirubin and moderate increase in transaminases and alkaline phosphatase. Weil's disease can mimic cholecystitis and have been reported cases of acute acalculous cholecystitis as a rare manifestation of leptospirosis in adults.
We present the case of a 42 year old man who was attended at the Internal Medicine ward of the Hospital “Angeles del Pedregal” because of clinical symptoms characterized by myalgias, arthralgias, headache and fever. During his stay he had a clinical picture, laboratory and imaging consistent with acalculous cholecystitis, he was underwent a surgical procedure and with suspicion of Weil syndrome we initiated management with doxycycline and the serology for Leptospira interrogans confirmed the diagnosis and evolved towards improvement. Weil's disease can mimic a clinical picture similar to acute cholecystitis, so it must be considered if risk factors for acquiring the disease.
