Neonatal Screening an Early Diagnosis for Congenital Hypothyroidism at “Dr. Aurelio Valdivieso” General Hospital in Oaxaca

ABSTRACT
Objective: To know the time lapse between birth, diagnosis, and initiation of treatment of congenital hypothyroidism (CH) in suspected cases reported per neonatal screening at the General Hospital “Dr. Aurelio Valdivieso” in Oaxaca.
Material and Methods: From January 1, 2006, to December 31, 2011, we studied 41 cases with CH detected by neonatal screening; we determined the site of origin of each patient and the health clinic where the sample screens were taken. The days that lapsed between the birth, confirmation, and initiation of treatment, and clinical manifestations of HC at the time of diagnosis were recorded.
Results: The mean age at initiation of treatment was 40.17 days. The cases in which treatment was started within 30 days were 13 (31.7%); those in which treatment was initiated between 30 and 60 days were 22 (53.7%) and those who started treatment after 60 days were 6 (14.6%). Patients with clinical manifestations at the time of diagnosis were 39 (95.1%). In one town, Santiago Suchilquitongo, 6 cases (14.6%) were diagnosed.
Conclusions: The difficulty of access to different parts of the State of Oaxaca contributes to delays in sample collection and the reporting of results of neonatal screening. The time lapse between birth and the initiation of treatment is higher than recommended by the technical guideline for the program of congenital hypothyroidism, and increases the risk of neurological damage in patients; hence, strategies must be designed to allow state-level diagnosis within 30 days of life of the patients.
