ABSTRACT
Large incisional and recurrent hernias require prostheses.
Hypothesis: Bovine pericardium bioprostheses prepared with glutaraldehyde (PBTG) have advantages over synthetic polyester protheses.
Objective: To determine ideal glutaraldehyde concentration to prepare bovine pericardium bioprostheses and to compare with synthetic polyester prostheses in abdominal wall reconstruction in rats.
Material and methods: Phase I. We studied the physical characteristics (tensile stregth, elasticity, and elongation) of PBTG prepared with different glutaraldehyde concentrations. We used stable microsystem TA XT2 analyze texture. Phase II. Eighty Wistar rats were used in the experiment; they were divided into four groups of 20 animals each (group A-D). Abdominal wall reconstruction at 1% PBTG (A, B) was compared to that of polyester sheet (C, D). (A, C) rat groups were killed after 3 week; (B, D) groups were killed after 3 months. Physical characteristics and histopathologic alterations of abdominal wall of all animals were studied.
Results: Phase I. PBTG prostheses at 1% had highest tensile strength. Phase II. With the polyester sheet at 3 weeks, tensile strength, elasticity, and elongation were highest, decreasing at 3 months (p ‹ 0.05); with PTTG, decrease was lower. All groups experienced inflammatory reaction and angiogenesis, synthetic sheet produced granulomes, and fibrosis was high with better prostheses integration. PBTG did not produce granulomes and at 3 months, calcifications occurred. PBTG prostheses did not show advantages in relation to polyester sheet.
