Laparoscopic cholecystectomy conversion, beyond the learning curve

ABSTRACT
Introduction: Laparoscopic cholecystectomy is the actual gold standard for the treatment of gallbladder disease. In the experienced surgeon conversion to open surgery relies mainly on clinical conditions of the patients.
Material and methods: Retrospective analysis was made on medical records of patients who underwent laparoscopic cholecystectomy at the Nuevo Sanatorio Durango Medical Center from January 2008 to January 2011.
Results: 707 patient files that underwent laparoscopic cholecystectomy by an experienced surgeon were analyzed from which 501 were female (70.8%) and 206 male (29.1%). There were 28 open surgery conversion cases (3.9%), from which 17 were associated with male gender (OR: 4.0 p = 0.0004), 41% due to anatomical variations. Independent risk factor for conversion was age above 55 years (OR: 2.17 p = 0.05). Acute cholecystitis was associated with conversion (OR: 2.84 p = 0.01), mainly by surgical treatment 72 hours after acute illness setting.
Conclusions: Conversion to open surgery was associated with acute cholecystitis and due to patient related factors Laparoscopic surgical treatment of acute cholecystitis in the early phase of the disease is preferred.
