Percutaneous dilational tracheostomy vs open tracheostomy. Experience at Hospital Angeles Lomas, Mexico

ABSTRACT
Background: Tracheostomy is a very common procedure. Open surgical tracheostomy, that is the conventional procedure, has evolved up to diminish the rate of complications. In 1955 percutaneous tracheostomy arises and has been gaining popularity among the otolaryngologists and critical care physicians as an effective measure in the managing of the airway.Objective: To report our experience in the accomplishment of percutaneous tracheostomy vs open tracheostomy in critically ill patients at Hospital Angeles Lomas.
Patients and method: A retrospective and comparative study which included all patients in the intensive care unit at Hospital Angeles Lomas who underwent open tracheostomy and percutaneous dilation tracheostomy from1999 to 2007. All patients had endotracheal intubation and had no history of tracheostomy. We reviewed directly each of the records.
Results: In open tracheotomy, we found eight cases of bleeding, eight cases of cutaneous infection, 13 cases of edema and three air leak. In three cases of percutaneous tracheostomy, we found bleeding, none by infection, five of edema and two cases of air leak.
Conclusions: Percutaneous dilation is the technique of choice to perform a tracheostomy, since it causes less postoperative complications.
