Twin birth: is it a safe option?

ABSTRACT
Background: The ideal way of birth twin pregnancies is not well established yet, since there is no level I evidence studies in the series published so far. The rate of obstetric complications in this group of patients and the heterogeneity of procedures make it more difficult the birth election.
Objective: To determine the safety of vaginal birth in twin pregnancies.
Patients and methods: We reviewed the records of the labor room at Hospital Angeles Lomas, from 2003 to 2010 and were recorded as cesarean or vaginal deliveries of twin pregnancies and compared each form of conclusion of the pregnancy.
Results: The total number of twin births was 299, equivalent to a rate of 29 per 1,000 live births. The number of twin births during the period was 35, which corresponds to 11.7% of twin pregnancies concluded this way. It was found that 31.4% of second twins vaginally born had 1-minute Apgar score below 7, compared with 10.8% of second twins born by cesarean section (p = 0.002), however, the 5-minute Apgar no significant differences (p = 0.4).
Conclusions: The vaginal birth of twins is not associated with adverse neonatal or maternal, when performed with adequate obstetric assessment.
