Use of plasma rich in growth factors to reduce the recurrence of nasopalate fistulas in patients with history of cleft palate

ABSTRACT
Background: Cleft palate is the most frequent congenital malformation of head and neck. In Mexico there are 3,521 new cases a year. About 20-30% of patients submitted to primary palatoplasty have fistula’s recurrence, requiring a new surgical event. This represents a high cost for the country, as well as increased risk for patient and life-quality worsening. Plasma rich in growth factors is an autologous hemoderivative that has showed to have benefits such as accelerated cell regeneration, neovascularization, osteogenesis, among others. Its use has not been reported in nasopalate fistulas repair.
Objective: To demonstrate that the use of plasma rich in growth factors reduces the nasopalate fistulas recurrence.
Patients and method: An experimental and prospective study was done from April 2008 to June 2009, with a total of seven patients, 10 nasopalate fistulas, which were closed by local mucoperiosteal flaps, placement of autologous bone graft mixed with the plasma rich in growth factors.
Results: With a following of 6-12 months, the complete closure of fistulas was demonstrated in 90% (p = 0.14), reducing the recurrence rate reported by other authors. Variables were studied to compare them with the closure; none of them resulted statistically significant.
Conclusions: The use of plasma rich in growth factors during the nasopalate fistulas closure is effective, safe and of low-cost for patient.
