Preoperative levels of natriuretic peptide in adult cardiac patients submitted to cardiovascular surgery

ABSTRACT
Background: The heart produces B-type natriuretic peptide (BNP) in response to wall stress. Levels of BNP can be used for a comprehensive evaluation of the patient's preoperative condition.
Objective: To report preoperative B-type natriuretic peptide values in cardiac patients and their intraoperative behavior.
Patients and method: An observational, cross-sectional study in cardiac surgical patients was done. Statistical analysis included measures of frequency, ROC curves and Mann Whitney, with alpha of 0.05.
Results: We studied 20 patients underwent to surgical myocardial revascularization (n = 10); valve replacement (n = 8) and combined procedures (n = 2). Ejection fraction of left ventricle was 49.2 ± 11.7, mean operative time was 4.6 ± 0.7, and 5.6 ± 0.9. The use of amines was 388.3 to 477.5 pg/mL, with a sensitivity of 83-92%. The group that showed postoperative complications had a B-type natriuretic peptide of 1,827 ± 991. The dose of amines ‹ 5 mcg/kg/h induced an average B-type natriuretic peptide of 1,610.86 ± 550.28.
Conclusions: The sample size was limited, but apparently there is a positive trend for preoperative B-type natriuretic peptide levels and use of amines and complications.
