ABSTRACT
Hemorrhoids are normal formations in the organism and when they’re asymptomatic doesn’t need any treatment. Hemorrhoids have an specific function in the continence mechanisms In about 80% of the cases, the hemorrhoidal disease is medical. In thrombosed external hemorrhoids the main treatment is simple hemorrhoidectomy. In the second grade hemorrhoids the Ideal treatment should be Barron ligation with the rubber band, In the 3
rd grade of the hemorrhoids with mucosal prolapsed and external component should be use macro high ligation with removal of the external component a conventional technique (Milligan and Morgan semi closed or Ferguson closed). In 3rd grade hemorrhoids with little external component and mucosal prolapsed. The specific indication is the circular mucosectomy. While in the same grade with thick external component one of the conventional techniques should be used nowadays with the new etiology concepts of the hemorrhoidal disease as a product of freely chronic prolapsing of the anorectals walls, we should be sure that even of the external component marked in the hemorrhoidal disease of the 2
nd, 3
rd and 4
th grade. The circular mucosectomy should be the gold standard in the treatment of these pathologies. For that reason, we propose a consensus of all the alternatives of the treatment, based on previously works, comparative results as well the contraindications of the treatment for the hemorrhoidal disease.
