Minimally invasive video-assisted thyroidcetomy: 10 years experience through the eyes of evidence

ABSTRACT
Introduction: Minimally invasive video-assisted thyroidectomy (MIVAT) was first described by Professor Paolo Miccoli in 1998. Even though several trials and series of patients provide enough evidence to support its routine use it is neither being practiced nor taught in our country.
Objective: To define the actual role for MIVAT within the general surgeons armamentarium based upon the best available evidence.
Materials and methods: A Medline search was performed for all randomized controlled trials using the MeSH term «thyroidectomy»; all abstracts were reviewed to determine compliance with the inclusion criteria as well as the level of evidence it provided.
Results: There were a total of 88 trials identified. Only those comparing conventional thyroidectomy (CT) versus MIVAT were included. MIVAT proved superior in cosmesis, caused less phonatory and swallowing symptoms and required less analgesic dosage than CT. MIVAT is comparable in terms of hypoparathyroidism CT but requires longer operating times.
Conclusions: The available evidence allows for the following recommendations to be issued: MIVAT is safer and provides better cosmesis for the treatment of selected patients (Grade A) and it is also suitable for the treatment of low to intermediate risk well differentiated thyroid carcinoma (Grade B).
