Squamous lesion in subjects practicing anal intercourse: comparison of cytological vs histological findings

ABSTRACT
Background: Infection with human papilloma virus (HPV) of the anus appears predominantly in homosexual. Cause intraepithelial lesions that lead to anal carcinoma.
Objective: To know the result of detection of precancerous lesions in subjects who practice anal intercourse.
Material and methods: Prospective, cross sectional of patients attending the Hospital de Especialidades Centro Medico Nacional La Raza. We studied homosexual and bisexual subjects with and without HIV/AIDS from February to October 2010. In each case it was done anoscopy under application of acetic acid and cytology of the anal transition zone. Biopsy of the lesions obtained of acetowhite zones or random if there was not visible lesions. We compared the results of histopathology (gold standard) with cytology, using simple frequency analyses.
Results: We studied 28 subjects, within an age range of 21 to 68 years. Twenty three were HIV positive. By cytology we saw 13 cases with squamous lesions, 10 with low-grade lesion (LSIL) and 3 high-grade lesion (HSIL). Three with squamous lesions of uncertain significance (ASCUS). Biopsy: 3 of 10 confirmed cases LSIL, all 3 with HSIL, and one case of ASCUS, which corresponded to LSIL. Biopsy showed 15 squamous lesions, nine with LSIL, 6 with HSIL. In 2 cases biopsy give us the etiology: histoplasmosis and cytomegalovirus as the cause of local ulcers.
Conclusions: Our preliminary data shows 15/28 squamous lesions caused by hpv , in two other subjects we could determine the precise etiology of local ulcers. Each one of the subjects with squamous lesion could be under treatment that prevents its evolution toward cancer. As has pointed earlier in the literature we saw the importance of proctologic examination in men who have sex with men.
