Soleous muscle flap to cover exposure fractures in the distal third of the leg

ABSTRACT
Open fractures of the distal tibia should be covered with flaps. Adequate coverage can be accomplished with the soleus muscle flap following within the standards established treatment of reconstruction of the distal limb. The soleus muscle flap has been widely used to cover defects of the middle third tibia, although it can reach the distal third in defects in lower extremities. The use of microsurgical flaps has been established as preferred, but the suggestion of soleus flap has not lost its value today, since not all patients meet criteria for microsurgical reconstruction. In this study we used the soleus muscle flap to cover tibia distal defects in three patients with exposure fractures, two with type Gustillo’s IIIb fractures, and in one patient with exposed osteosynthesis material achieving adequate coverage of defects and reintegration to work. The soleus muscle flap is easy to make. It has an adequate vascular supply; it covers the defect properly, and produces a good functional and aesthetic result.
