Efficacy and tolerability of mitomycin in the nasal synechia treatment

ABSTRACT
Background: Among otorhinolaryngological surgical interventions, especially those of cavities, stenosis or synechia are frequent, whose origin may be congenital or acquired, but most of them are iatrogenic.
Objective: To assess the efficacy and tolerability of mitomycyin vs traditional method (placement of dental wax on nasal cavity) in order to avoid relapse in patients with nasal synechia.
Patients and methods: A prospective and comparative study with 40 patients divided into two groups was done. Study group (n = 20) received topical mytomicin (0.5 mg/mL) after the synechia resection; group 2 (n = 20) received conventional treatment (placement of pink dental wax in the nasal cavity during seven days). All patients were submitted to nasal synechia resection as a consultation procedure, with local anesthesia. Relapse incidence and adverse effects were assessed at three days and at first, fourth and eighth weeks of procedure.
Results: Average age-of-onset was of 31 years for both groups with predominance of female patients (55%); the most frequent site of presentation was the right nasal cavity. The main etiology was nasal trauma followed by rhinoseptoplasty and septoplasty. The test group had a recurrence of 10% compared with 30% in the control group, with complete tolerance in the evaluation group.
Conclusions: Topical mitomicyn application should be considered an alternative method in patients with nasal synechia resection, because it decreases nasal obstruction and secondary symptoms; it is also a regimen with good tolerability.
