Is it still a therapeutic challenge gastroschisis in our midst? Prevalence, Complications, and Mortality Associated malformations

ABSTRACT
Introduction: The objectives were to determine the prevalence, complications and mortality associated malformations in newborns with gastroschisis.
Material and Methods: We did a retrospective study of patients with gastroschisis treated from 1 October 2001 to September 31, 2004. To obtain the prevalence excluded patients not born in our hospital.
Results: 42 patients with gastroschisis were treated at our institution in the study period. The average prevalence was 8.6 per 10,000 live births. The average gestational age was 37.5 weeks and birthweight was 2354g. The time between birth and surgery was 11.7 hours, performing primary closure in 45.2% of patients. Sepsis was a complication in 76.1% of cases. Associated malformations were present in 11 patients (26.2%). The mortality in our series was 42.9% with sepsis the leading cause of death in 83.3% of cases.
Discussion and Conclusions: The prevalence of gastroschisis was twice that reported in other series. Sepsis is the most common and important complication for mortality. Mortality was higher with statistical significance associated with low birth weight, prematurity, low Apgar score at one minute, time between birth and primary surgery, use of assisted ventilation, number of surgeries and intestinal atresia. Mortality is higher than that reported in Anglo-Saxon literature and similar to those reported in the Latin American series.
