Laparoscopic cholecystectomy in children Experience of a tertiary Hospital

ABSTRACT
Introduction: Since the first report of laparoscopic cholecystectomy in adults conducted in Germany in 1985 by Erich Muhe and later in June 1990 at Vanderbilt, when Holcomb and colleagues performed the first laparoscopic cholecystectomy in children are few reported significant series in the world literature and in our country. So our aim is to show the experience in laparoscopic cholecystectomy in children in a tertiary hospital.Material and Methods: We conducted a retrospective study in January 2003 to February 2007, patients undergoing laparoscopic cholecystectomy. Assessing factors such as age, weight, sex, indication for surgery, hospital stay, surgical time, surgeon performing the procedure (medical resident or base), attachment procedures, complications and current status.
Results: The study included a total of 94 patients in the study period, 69 female and 25 male. The mean age was 10.5 years. The main indication was gallstone cholecystitis in 86%, other acalculous cholecystitis, dyskinesias vesicular and choledochal cyst. We obtained a conversion rate of 3%, hospital stay was 2-6 days. Follow-up was one week.
Discussion: It has been reported in the literature that the most common cause of cholecystectomy in pediatric patients is cholelithiasis conversion in our series was 3%, requiring exploration of the bile duct and one for bleeding. The laparoscopic approach remains the cornerstone of treatment for gallstones, with a reproducible technique with high success rate, even when performed by pediatric surgeons residents, under the tutelage of an experienced surgeon in the area.
