ABSTRACT
Background: Non-Hodgkin’s lymphoma (NHL) of the B-cell type is the second most common neoplasm among patients with human immunodefi ciency virus infection (HIV) and AIDS. The high frequency of extranodal involvement is one of the major clinical characteristics of AIDS-associated NHL. Highly active antiretroviral therapy (HAART) has diminished opportunistic infection-related morbidity and mortality rates as well. A different scenario may be observed in regard to NHL. Liver lymphoma is an uncommon neoplasm in these patients; it can be primary or secondary to other sources. NHL compromises the liver in 20% of patients while Hodgkin’s disease involve the gland in 5% to 8% of the cases. Ultrasonography and computarized tomography (CT) scans may be helpful in the diagnosis of focal hepatic lymphoma. It usually presents with single or multiple large hepatic masses and lymph node involvement. Image-guided fi ne-needle biopsy with histopathology of the liver lesions is the gold standard for the diagnosis of hepatic lymphoma.
Objective: To describe the case of fi ve patients with AIDS and liver involvement by lymphomas.
Patients and method: Diagnosis was confi rmed by CT scan or ultrasonography guided hepatic biopsy and histopathology.
Results: Four of the patients presented as multiple and large hepatic masses. Histopathological fi ndings include three cases of diffuse large B cell lymphoma (DLBCL), one of DLBCL with plasmablastic differentiation and one of Hodgkin lymphoma.
Conclusions: Timely diagnosis followed by proper treatment including chemotherapy combination plus antirretroviral therapy may modify the poor prognosis of these patients.
