Extralobar pulmonary sequestration A case report and literature review

ABSTRACT
Introduction: Congenital lung malformations are rare at any age, the incidence is between 0.15% to 1.7%. Pulmonary sequestration is defined as a non-functional cystic lung tissue that has no communication with the normal bronchial tree and receives blood supply from anomalous systemic arteries, this anomaly was first described in l861. presents a case of extralobar pulmonary sequestration.
Case report: A male patient aged 4 years admitted to our hospital for surgical treatment of pneumothorax and bronchopleural fistula of the right middle lobe. He was treated as pneumonia and congenital lobar emphysema. His medical history included recurrent patterns of frequent lower respiratory tract infection. In plain chest X-ray study of admission showed right lung parahilar hyperaerated air. Elective thoracotomy was performed revealing an extralobar pulmonary sequestration of lower lobe. Abnormal systemic artery providing blood supply was paravertebral branch of the thoracic aorta.
Discussion: pulmonary sequestration involves the lower lobe in 97% of cases. This case presented in the middle lobe, causes repetitive respiratory infection. When suspicion of pulmonary sequestration is recommended to make the diagnosis of chest CT, aortography and pulmonary angiography.
Conclusion: Intra or extralobar pulmonary sequestration is a rare disease. We recommend early surgical resection should be conservative separating the normal lung parenchyma.
