Subpccipital concentric craneotomy surgical results en 14 cases

ABSTRACT
The approaches to posterior fossa have surgical evolution since initial use by Krause in the last century and with risks for venous sinuses injury, the suboccipital concentric craniotomy is a variant with new description for minor complications.
Clinical material and methods:were 14 consecutive patients that we use the suboccipital concentric craniotomy, 12 in midline and 2 lateral, by surgical posterior fossa lesions, in the Departament of Neurosurgery from the Institute Nacional of Pediatry.
Results. The patients were 9 man and 5 women, of 2 to 15 years old age, media 7.93. Thirteen lesions were neoplasms (6 medulobllastomas, 4 pineal germinoma, 3 pilocytic astrocitoma) and one vascular disease; of the 14 procedures: 12 was in the midline, with surface in the initial craniotomy 7.6cm
2 and the complete 37.3 cm
2 and 2 lateral, the surface in the craniotomies initial and complete 4.5 cm
2 and 15.5 cm
2, respectively. Without complications. The total follow up was 62 months and media 4.4 months. Discussion. Actuality the approach is with suboccipital craniotomy or craniectomy, with risks by complications to venous sinuses and the variant suboccipital concentric craniotomy in this case series, without these risks.
Conclussion: we discuss the suboccipital approach and the evolution by the craniectomy and the preferred use for the craniotomy and with the “concentric” variant that improvement the surgical technique
