Results of intertrochanteric osteotomy for the management of the late sequelae of slipped capital femoral epiphysis

ABSTRACT
Introduction: The management of deformity after a severe slipped capital femoral epiphysis (SCFE) is controversial due to a lack of long-term follow-up studies. The objective of this study was to determine the clinical and radiographic result of patients treated with an intertrochanteric osteotomy for the late sequelae of a SCFE.
Material and methods: 13 patients (14 hips) with sequelae of a SCFE treated with an intertrochanteric osteotomy were evaluated clinically and radiographically. The mean age at the time of operation was 15.5 years; mean follow-up was 10.2 years (range 5.9 to 18.2). The PODCI scale was used to evaluate the overall outcome and the Iowa Hip and Southwick Scores were used to evaluate specific hip outcome.
Results: The mean PODCI score was 90.75 (range 84 to 95). The mean Iowa Hip Score was 84.53 (range 58 to 96). The Southwick Score includes scales for pain, limp, range of motion (ROM) and the radiographic result; according to these scales: for pain there were 10 patients with an excellent or good result, 2 with a fair result and 1 with a poor result; for limping there were 6 patients with an excellent or good result, 3 with a fair result, and 4 with a poor result; for ROM there were 8 patients with an excellent or good result, 1 patient with a fair result and 4 patients with a poor result; radiographically there were 8 patients with an excellent or good result. No patients developed degenerative joint disease or avascular necrosis.
Conclusion: An intertrochanteric osteotomy allows the correction of certain deformities seen after a SCFE with good clinical and radiographic results and a very low incidence of complications.
