Surgical Treatment for Constipation

ABSTRACT
Constipation is a symptom that is usually present as a manifestation of associated diseases, it is the cause of 4-28% of medical consultation for gastroenterological diseases in the United States. The diagnostic approach of constipation is aggressive and extensive in order to exclude any underlying pathology, structural or functional alteration of colon or pelvic floor. Once excluded any alteration, constipation is considered as idiopathic and the possibility of surgical treatment to solve it is also considered. The present study shows the experience and outcome of surgical treatment for constipation in the Coloproctology Service at the Hospital de Especialidades del Centro Médico “La Raza” del Instituto Mexicano del Seguro Social. Twenty nine patients (5 male, 24 female) were surgically treated, severe chronic constipation and altered colonic transit was confirmed in all cases. Thirty eight procedures were performed among them: 20 full-thickness rectal biopsy, left-sided hemicolectomy in seven cases and subtotal colectomy with ileo-proctostomy in 11 patients. This last procedure solved constipation in all cases (100%), left-sided hemicolectomy in 2 (28.5%) and full-thickness rectal biopsy in 3 cases (15%). An exhaustive study of constipation is suggested in order to optimize the selection of candidates for surgical treatment.
