Uso del droperidol como agente antiemético por vía epidural en combinación con morfina para el control del dolor

ABSTRACT
Objetive. To evaluate the effectiveness of droperidol as antiemetic agent administered epidurally along with morphine sulfate to manage postoperative pain.
Material and methods. 40 female patients divided randomly in two groups of 20 each, ASA I-II, aging between 20 and 50 years, body weight from 60 to 70 kg,post operated of abdominal hysterectomy with continuous epidural anesthesia. Epidural postoperative analgesia was installed after surgery. Patients in group 1 received an initial 30 mcgrs/kg bolus of morphine plus a continuous infusion of 50 mcgrs/kg/day using an electronic PCA CADD PRIZM pump. Patients in group II received an initial 30mcgrs/kg bolus of morphine plus an initial 2.5 mg bolus of droperidol,and continuous infusion of 50 mcgrs/kg/day of morphine along with 2.5 mg/day dose of droperidol through an electronic PCA CADD PRIZM pump. In both groups were assessed: heart rate, respiratory rate, blood pressure non invasive, pulse oximeter, visual analogue scale (VAS), modified Bromage, Ramsay scales and basal side effects were assessed at 1, 3, 6, 12, 24, 36, 48, 60 and 72 hours.
Results. Group 1 the basal VAS was 2.3 on average, in group 2 the basal VAS was 2.5. In both groups the hemodynamic parameters were acceptable, Ramsay sedation scale was 2.5 on average, however there was a significant statistical difference. Side effects: vomiting 50%, nausea 95%, urinary retention 2.5% and pruritus 2.5% mainly in group 1.
Conclusion. droperidol administered epidurally along with morphine sulfate is a good agent to avoid nausea and vomiting as side effect of morphine when applied as sole agent.
