Secondary Testicular Tumors. Experience in a Third Level Hospital and Literature Review

ABSTRACT
Less than 5% of testicular neoplasms are secondary. Their origin is hematological and less frequently other solid tumors.
Objective and methods. To describe our experience in an observational and descriptive study. We analyzed secondary testicular tumors at our institution during the last 20 years.
Results. A total of 209 malignant testicular tumors were identified. Seven (3.34%) were secondary tumors, six were studied as testicular tumors and one was an incidental finding at orchiectomy for prostate cancer treatment. We found four cases with lymphoma, one with leukemia infiltration and two with solid tumor metastasis. The average age at presentation was 51 and half had a previous tumor history. In the autopsy group, testicular metastasis was found in 9 cases.
Discussion. The most common secondary testicular neoplasm was lymphoma and leukemia, followed by solid tumors. The possible metastatic routes are: vascular, lymphatic, spermatic vessels, retrograde (by eyaculatory ducts), and local extension.
Conclusion. Germ cell tumors are the most frequent testicular tumor. Older patients, however, with previous cancer history should have testicular metastasis excluded.
