Validation of a pediatric multiple organ dysfunction outcome score (eppdom)

ABSTRACT
Introduction: The mortality in Critical Care Units is important influenced by the multiple organ dysfunction. In our Service the incidence of SDOM is 32% with mortality of 32%, which represents the 68% of the general mortality of the PCCU. The objective of the study was to valid the pediatric prognostic of multiple organ dysfunction score (EPPDOM).
Material and methods: Prospective study of cohort that include all the patients which presents SDOM along of their permanence in the Service. Statistics tests: t of Student, ROC analysis and Hosmer-Lemeshow test.
Results: 257 patients was included 82 of that presents multiorganic dysfunction. The ROC analysis report an ABC of 0.82 when beginning the disease, 0.74 on the 3 day, 0.89 at day 5 and 0.84 in the critic day. The calibration test report p = 0.35 in the initial day, p = 0.59 on the day 3, p = 0.41 on the 5 day and p = 0.76 in the critic day.
Discussion and conclusions: The score predict the prognosis of the child with SDOM show accurate level of discrimination and calibration. EPPDOM is the only score that evaluate 7 organic systems and the only that include demographic epidemiological variables with prognostic value.
