Deep neck abscesses: etiology, pathogenicity, morbidity and mortality

ABSTRACT
Introduction: Deep neck abscesses incidence has been increased due to difficulty for its diagnosis and treatment, the virulence of germs, the indiscriminate use of antibiotics, and the associated pathology.
Objective: To know the etiology, pathogenicity, morbidity and mortality of deep neck abscesses in a polyvalent intensive care unit (ICU).
Design: Retrospective study.
Setting: A polyvalent ICU of a tertiary care hospital of Mexico City.
Patients: Thirteen patients (seven women, mean age 39.2 ± 14.7 years and six men, mean age 49.1 ± 16.6 years) admitted to a polyvalent ICU during a three-years period with diagnosis of deep neck abscess were enrolled.
Interventions: None.
Results: The source of infection was: dental 61.5%, Ludwig’s angina 23.1% and insertion of jugular catheter. The pathologies more frequently associated were: primary 46.4%, diabetes mellitus 38.4%, medicamentosus agranulocytosis 7.6% and dermatomyositis 7.6%. The duration of clinical manifestations was 9.9 ± 5.3 days, ICU stay 18 ± 9.43 days and mortality rate 23%.
Conclusions: Deep neck abscesses are a serious life-threatening infections, specially in compromised hosts and the success of its treatment is related to early diagnosis.
