Results of revascularization for rescue of pelvic extremities in fifty patients of the state of Oaxaca

ABSTRACT
Objective: To report the first results into our state from patients underwent to limb salvage revascualarization within a year period.
Patients and methods: From may 2003 to may 2006, fifty bypasses were performed for limb salvage in fifty patients. Thirty one patients (62%) were female and nineteen (38%) were male, with a mean age of 71 years. Eighty percent had diabetes mellitus, from these forty two percent had hypertension, Hypertension alone 18%, obesity 8% smokers 18%, coronary disease 10%, end-stage renal disease 4%, multiple sclerosis 2%. Forty six patients (92%) had critical limb ischemia, rest pain (8%).
Results: Forty eight patients underwent reverse greater saphenous vein revascularization, PTFE (1), composite PTFE- arm vein (1). By pass to infrapopliteal out flow vessels were: paramaleolar vessels 56%, popliteal below knee 40%; by pass to superficial femoral 4%. Failed graft 14%. Operative mortality rate (2%). Primary graft patency (30 days) 84%, six months 84%, one year 80%, three years 78%.
Conclusions: Saphenous vein is an excellent conduit when available. These results justify the routine use of arterial reconstructions for patients with critical limb ischemia.
