ABSTRACT

We report a case of a 21 years old man with diagnosis of secondary hemophagocytic syndrome caused by tuberculosis. He had a five month history of weight loss, fever and jaundice. He was admitted in the ICU because of respiratory distress requiring mechanical ventilatory support. He had bicitopeny, increased liver enzymes with hepatocellular pattern, and hepatoesplenomegaly. Tuberculosis was found in lungs and bone marrow by baciloscopy and bone biopsy. Severe hemophagocytosis was found in a bone marrow aspirate. Both diseases were treated with antifimic drugs and immunoglobulin, obtaining an adequate response. The hemophagocytic syndrome is a rare disease; it has an immunologic origin with diverse etiologies, and a high mortality. The infectious etiology is the least of all, and tuberculosis has been reported just in one case as a secondary cause of hemophagocytic syndrome.
