A Reliable Instrument to Evaluate Clinical Aptitude in Family Medicine Residents

ABSTRACT
Objectives: Our aim was to construct and validate an instrument able to evaluate clinical aptitude in first-year family medicine residents.
Materials and methods: We carried out a descriptive study in Family Medicine Centers 66 and 57 in Xalapa and Veracruz, Mexico. A test of 562 questions based on actual clinical cases concerning family medicine services, internal medicine, pediatrics, and gynecology-obstetrics was carried out on medical residents, allowing them three possible answers: true; false, and don’t know. Clinical aptitude was categorized into three levels: very high and high; medium, and low and very low. Reliance on physicians was evaluated using the Kuder-Richardson test, while the Pérez-Padilla and Viniegra Method was used to explain random answers. Experts in each of the previously mentioned areas analyzed construct validity. Finally, we utilized the Mann-Whitney U test to compare the results between the two institutions.
Results: A reliability of 0.8 was obtained. There were no significant differences between the Xalapa and Veracruz Family Medicine Centers. Clinical aptitudes obtained included low and very low.
Conclusions: The evaluating instrument was valid and reliable, and found low and very low clinical aptitudes in first-year family medicine residents.
