Treatment of femoral diaphyseal fractures by means of distal femoral nail (DFN) in patients with hip arthrodeses

ABSTRACT
Femoral shaft fractures in patients with previous ipsilateral hip arthrodeses is one of the most challenging complications to treat because the long lever arm that represents the arthrodesed limb. Plate osteosynthesis is the standard treatment of this fractures with the inconvenience of requiring a broad bone exposure for achieving stable fixation, satisfactory results between 50 and 70% are reported in most articles. Treatment of this pathology with distal femoral nail (DFN) offers biological and biomechanical advantages in comparison with plate fixation by preserving the fracture hematoma, less surgical aggression and greater resistance to mechanical stress. We report two cases of femoral shaft fracture in patients with ipsilateral hip arthrodeses treated by close reduction and internal fixation with distal femoral nail (DFN), a three years follow up, achieveing bone union in twelve weeks without any postoperative complications, no residual knee joint pain and a complete recovery of walking capability.
