Estudio comparativo entre diferentes pruebas de valoración de la vía aérea para predecir la dificultad de la intubación en paciente adulto.

ABSTRACT
Aim of the study: Comparison between six airway assessment methods to establish which one is the best to predict a difficult intubation.
Materials and methods: Time-course followed, comparative and prospective study. 199 adult patients, both sexes, physical status I-IV, presented for elective surgery under general anesthesia were studied. In each one, six airway assessment methods were performed during the preoperative visit. Once in the operating room and after induction of anesthesia, the laringeal view was graded during laringoscopy according to Cormack- Lehane´s classification. The association between each airway assessment method and difficulty to intubate was evaluated with chi square test, p‹0.05 was considered significant. The reliable diagnostic analysis of each aiway assessment method was done according to the Bayessian theorem.
Results: From the 199 patients studied, 11 presented a difficult intubation. The sternomental distance and mouth opening had the highest sensitivity, while the protrusion of the mandible and Bellhouse-Doré test had the highest specificity. The Bellhouse-Dorè test and sternomental distance had the highest positive predictive value. The sternomental distance and mouth opening had the highest negative predictive value.
Discussion: The sternomental distance proved to be the best airway assessment method to predict a difficult intubation.
