Arthrodesis with intramedullary nailing in post-traumatic arthrosis of ankle and hindfoot

ABSTRACT
Introduction. The post-traumatic ankle arthrosis, is one of the sequels that more frequently is observed, later to lesions like ankle fractures and to fractures of the pylon tibial, for what pain of light intensity is presented, until severe that the pattern of the march alters and therefore the displacement capacity, what rebounds in the socioeconomic life of the patients. They exist 30 surgical techniques approximately to achieve an arthrodesis of the ankle, each one with their approaches and necessary synthesis material. Objective. To give to know a treatment option in the handling of the post-traumatic ankle arthrosis and hindfoot. Material and methods. It is a prospective, longitudinal study, of clinical, experimental rehearsal, carried out in the General Hospital Of Mexico SS OD, of June of 1999 to January 2004. It is had 21 cases, 13 women and 8 men, with post-traumatic ankle arthrosis, ages of 19 to 75 years whose predominant symptom was the pain. They were carried out approaches, medial, lateral and plantar, for stabilization with nail transcalcaneous of 9 mm of diameter and 200 mm of longitude blocked with fasteners 4.5 was used. Results. We find that in the etiologys they were 4 exposed fractures and 17 closed, receiving initial treatment; surgical 6 patients, conservative 8 cases and without treatment 7 cases. The arthrodesis was achieved in all the cases and the absence or important decrease of the pain. The minimum pursuit was of 3 months. As complication a blockade flaw was presented in the fasteners distals. Conclusions. The arthrodesis of the ankle and hindfoot by means of the blocked fixation intend as a sure and effective method in patient with post-traumatic arthrosis obtaining good clinical results and radiographics.
