Prevention of supine hypotension in the pregnant submitted to cesarean under BPD preloading vs rapid load

ABSTRACT
Objective: Determine if one preloading the 1,000 mL the crystalloid solution administered 10 min before the installation of peridural block for cesarean section is one effective method for prevention the supine hypotension, comparative with rapid loading the moment of the block installation.
Method: Longitudinale, prospective, comparative, experimental and clinic study, with a total 60 patients, distributed in 2 groups, with 30 patients each one.
Group A: 1,000 mL received a preload of crystalloid solution like total volume, administer 10 min. before the BPD installation.
Group B: volume rapid load proportion to same, administer the moment to BPD installation.
Results: The inside of hypotension was similar in both groups. Preload (6 patients 20%) Rapid Load (10 patients 33.3%). The decrease of systolic arterial pressure (SAP) to less than 30% of the basal was for group A (104 ± 12.3 mmHg)
vs Group B (103 ± 12.1 mmHg) obtaining a p › 0.05. The SAP ‹ 90 mmHg was equal en both groups, not thus in the decrease of 30% occurred in 36 occasions group B
vs 16 in the group A.
Conclusion: The volume preload administer to patients undergoing to cesarean section under BPD not decreased the incidence and severity of supine hypotension compared to the rapid load to the moment to the same installation.
