Iatrogenic vascular injuries

ABSTRACT

Introduction: In today medicine, invasion of the cardiovascular system is required in order to: accomplish several medical treatments and surgical procedures. These procedures can result into an iatrogenic vascular injury.
Objective: To present our experience on vascular iatrogenic trauma; to evaluate it frequency; complications; importance of complementary procedures during this treatment and permanent damage frequency.
Patients and methods: From 1982 to 2005 we have had 19 patients suffering from iatrogenia; 12 male and 7 female patients, with ages from 19 to 70 years; 5 minors ten years old. These patients were grouped on: 7 patients with Iatrogenic by omission and 12 patients suffered from Iatrogenic by commission.
Results: Predominant data shows: pain and absence of pulse at the affected member and patients went into shock. 58% of the patients were intervened within the first 8 hrs. 21% after 8 hrs and 21% where not surgically intervened. Patients with omission injuries had: a mortality rate of 43%; one amputation 14%; two permanent neurological sequels 29% and only a patient with normal evolution 14%. Patients with injuries by commission we had: 92% of the patients with normal evolution; only a single amputation that represented 8% and patients showed no permanent injuries or death.
Conclusions: Injuries by commission are generally identified as soon as they take place, this allows an immediate intervention thus avoids following complications, Patients generally evolve in a normal fashion and without sequels. Injuries by omission are not initially suspected or identified for a period of time, these prolong hospitalization and require surgical procedures that were not intended; increases mortality rates and permanent sequels.
