New classification for the traumatic hip luxation

ABSTRACT
Between January 1992 and August 1998, 24 patients with fracture-dislocation of the hip and 13 with pure dislocation were treated at our Hospital. We present a new classification system that includes five types according to the acetabular fracture stability and complexity following Letournel’s criteria: Type I simple dislocation, type II dislocation associated to stable elementary fractures, type III associated to unstable elementary fracture, type IV to a complex fracture and type V to a femoral head fracture. Treatment included close reduction for all the isolated dislocations and internal fixation for stabilization for most fracture-dislocations. All injuries were classified according to the new proposed system and the results were evaluated according to the Merle d’Aubigné and Postel, modified by Matta criteria in excellent, good, fair and poor. We present the evolution up to six years. Better results were found after pure dislocation in which reduction time was within the first five hours after trauma. We emphasize the importance of early mobilization without wear bearing for 12 weeks, and the relations with several factors associated to the initial trauma and the final outcome.
