diponectin: The adipose tissue beyond of the inert reserve of energy

ABSTRACT
The adipose tissue in addition to its participation in the energy homeostasis has an important endocrine function as source of a number of biologically active cytokines. Adiponectin is an adipocyte specific protein that circulates at concentrations 100-500 fold higher than other hormones and cytokines, and in contrast to all adipocytokines its serum concentration is markedly reduced in obesity, type 2 diabetes, coronary artery disease, and leading to reduction in insulin sensitivity. Adiponectin levels negatively correlate with glucose, insulin, triglycerides, and BMI and directly correlate with HDL cholesterol and insulin-stimulated glucose uptake. Adiponectin circulates as either globular lower molecular weight fragments or larger multimers of high molecular weight. The high molecular weight form is a better predictor of insulin sensitivity and glucose tolerance than total adiponectin. It has been shown that low plasma adiponectin levels are an independent predictor of type 2 diabetes. Adiponectin has been reported to have direct antiatherosclerotic effects such as inhibition of the expression of adhesion molecules and foam cell formation. This paper aims to review the most recent progress in adiponectin research and to emphasize its clinical implications.
