nfluence of clinical data in the diagnostic accuracy of the fine needle aspiration biopsy of the thyroid nodule

ABSTRACT
Objective: To determine if clinical criteria, consigned in the cytologic files, influence in the diagnostic accuracy in the Fine needle aspiration biopsy of the thyroid nodule. Place: Third level health care hospital. Design: Retrospective, transversal, comparative study. Methods: We compare the diagnostic value in 137 cytologic files of patients with FNAB of the thyroid nodule and who had been operated. Two groups were formed depending if the former clinical data were consigned or not in the cytologic files Group 1 (G1) with no clinical data, and Group 2 (G2) with clinical data. We analyzed the following clinical data: clinical diagnosis, age, sex, evolution time, nodule vs diffuse growth, size, functional state and positive or negative MIBI and compare the diagnostic accuracy between both groups. Statistical analysis: Sensibility, specificity, diagnostic accuracy, positive and negative predictive value (PV), and positive or negative Likelihood ratio (LR). Results: The population studied consisted of 125 women and 12 male, average ages of 45 years. We found 83 (60.5%), and 54 (39.5%) were malignant. The sensibility of the FNAB in G1 was 84.6%, specificity 97.7%, the diagnostic accuracy 91.8%, PV+ 95.6%, PV- 89.4%, LR+ 35.1 and LR- 0.08. The sensibility of G2 was 87.5%, specificity 100%, accuracy 95.2%, LR+ 0 and LR- 0.125. When we compared both groups we found no statistical significance between accuracy and diagnostic FNAB (p > 0.05). Conclusion: The clinical data not influence the diagnostic accuracy of the FNAB of the thyroid nodule.
