Nebulized dexamethasone versus intravenous dexamethasone for the treatment of acute asthma in pediatric emergency department

ABSTRACT
Introduction: Acute asthma is an important problem of public health and is associated to hospitalization specially in autumm and winter.
Objective: To evaluate the efficacy of inhaled dexamethasone
vs intravenous dexamethasone administration in children with acute asthma.
Design: Prospective study.
Setting: Emergency room (ER) of a general hospital of Naucalpan, State of Mexico, Mexico.
Patients: A total of 120 children (86 boys and 34 girls, mean age 8 ± 3 yrs [range 6-16 yrs]) with acute asthma admitted to an emergency room during a one-year period. Sixty patients (group A) received nebulized salbutamol + dexamethasone every 60 minutes (four doses) and 60 patients received nebulized salbutamol (every 60 minutes, four doses) + a single doses of intravenous dexamethasone.
Results: A clinical amelioration was observed in 49 patients of group A and 24 patients of group B (p ‹ 0.001. ER stay was 8.02 hours in group A and 36 hours of group B (p ‹ 0.001), SpO
2 was 92% in group A 86% in group B at 240 minutes (p ‹ 0.001) and maximal expiratory flow at 75% of expired vital capacity (FEF 75%) was 82% in group A and 75% in group B (p ‹ 0.001).
Conclusion: Inhaled dexamethasone + salbutamol is an promissory treatment in children with acute asthma in the emergency room.
