Subtotal cholecystectomy with Luper technique: an alternative management for a difficult cholecystectomy

ABSTRACT
Introduction: laparoscopic cholecystectomy is a very common and routine procedure, with more than 300,000 cholecystectomies performed in Mexico each year. Of these cholecystectomies, there is a large number of which are considered difficult cholecystectomies. These difficulties may arise due to the altered anatomy of the gallbladder, nearby organs or patient comorbidities, which increases the risk of complications and prolongs surgical time.
Objective: to describe a subtotal cholecystectomy technique in which an endoloop is used to ligate the gallbladder remnant.
Material and methods: a subtotal cholecystectomy technique was created in which a retrograde dissection of the gallbladder is performed to subsequently ligate the remnant using an endoloop. A retrospective study was carried out in which patients who were treated using this technique were evaluated.
Results: nine subtotal cholecystectomies were performed with this technique at the time of writing this publication, in which the patients evolved favorably, all of them were monitored using a Penrose drain. There were no complications recorded in the postoperative period.
Conclusions: it is important to note that, although the initial results of the technique are promising, they are based on a relatively small sample of patients. Therefore, an extension of the study with a larger sample is required to validate and generalize these results.
