Chronic disseminated candidiasis (CDC) due to Candida tropicalis in acute myeloid leukemia. Case presentation and literature review

ABSTRACT
Candida tropicalis is an asexual dimorphic yeast, non-albicans species most frequently associated with candidemia
and invasive candidiasis, with chronic disseminated candidiasis being an invasive form, characterized by hepato-splenic
involvement.
Clinical case. 56-year-old woman diagnosed with acute myeloid leukemia treated with second-line chemotherapy,
admitted for salvage chemotherapy, presenting neutropenia and fever on day + 11, broad-spectrum antibiotics were
started, persisting with fever, adding painful syndrome abdominal, abdominal tomography was performed with the
presence of hepatosplenic fungomas, amphotericin b deoxycholate was started empirically, isolation of pan-sensitive
C. tropicalis in blood cultures, adjustment to echinocandins with clinical improvement after 72 hours. In only 5% of
cases of invasive candidemias are they proven, so clinical suspicion is essential for the early initiation of antifungal
treatment, with the aim of reducing the risk of lethal outcomes.
