Prevalence of early prone positioning in patients under invasive mechanical ventilation with COVID-19 in a center in Mexico

ABSTRACT
Introduction: various studies on patients with severe acute respiratory distress syndrome (SARS) undergoing invasive mechanical ventilation in the prone position have confirmed improvements in oxygenation and mortality. The main objective was to determine the prevalence of early use of the prone position in patients undergoing invasive mechanical ventilation with COVID-19.
Material and methods: the records of patients diagnosed with COVID-19 who required invasive mechanical ventilation from March 2020 to March 2022 were consulted. A descriptive analysis was performed using measures of central tendency, as well as bivariate analysis and logistic regression models to determine the risk of mortality.
Results: a total of 456 patients were included, of which 348 were male, with a median age of 57 years. In the bivariate analysis, statistically significant differences were found in the variables of age, days on ventilation, leukocytes, D-dimer, absence of complications, among others; in logistic regression, only the days on ventilation and age showed significance.
Conclusions: the use of the prone position during the first 24 hours after the initiation of invasive mechanical ventilation in patients with SARS-CoV-2 was common in treatment; however, a significant relationship between its use and 30-day mortality was not established.
